Screening for hypertension

Routine screening is essential to identify cases early, initiate timely treatment, and reduce cardiovascular risk. This guide covers current evidence-based screening protocols.

When to screen
  • All adults ≥ 18 years should be screened at least annually.
  • Screen more frequently if elevated risk: age >40, obesity, family history, prehypertension.
  • Children/adolescents: Annual screening beginning at age 3.
How to screen
  • Measure BP at least twice in controlled setting with validated device.
  • Use correct cuff size. Have patient relax 5+ minutes before measurement.
  • Average ≥2 readings. Confirm elevation with repeat visits before diagnosis.
  • Out-of-office monitoring also recommended to confirm.
Referral Indications
  • Systolic ≥130 or diastolic ≥80 mmHg
  • Prior diagnosis without adequate control
  • Symptoms suggesting secondary hypertension
